  3. Type in the last four digits of your social security number into the field provided.
  4. Input your date of birth in the fields provided.
  5. Click on the “Continue” button.
  6. Continue to move forward and provide all the required information to create your new online account.

  1. Visit: https://www.htbenefits.com in an internet browser on your device.
  2. Scroll to the bottom for the login section. NOTE: Your Plan Source Username consists of: 
    • First initial of your First Name;
    • First six (6) letters of your Last Name; (Or, your entire Last Name if it is less than six letters.) 
    • Last four (4) digits of your SSN.
    • Example: Mike Kealoha, whose SSN is 000-00-4567, would have a login of MKEALOH4567
  3.  Your Plan Source Password is your birthdate in this format, YYYYMMDD (unless you have previously changed your Plan Source password). Example: a birthdate of February 9, 1985 would look like this: 19850209
  4. Proceed to move forward and provide all the required information to create and set up your new online benefits account.
