Follow these easy steps below if you are a NEW Patient who needs to create and activate a NEW online account.

  1. Visit: www.labcorp.com/patientinfo on your device.
  2. Next, click on the “Register” button.
  3. Then, type in your name in the fields provided.
  4. The next step is optional: input your gender.
  5. Type in the Date of Birth of the patient on file in the fields provided.
  6. Type in your address in the fields provided.
  7. Type in a contact phone number in the field provided.
  8. Create a password and type it in the fields provided.
  9. Type in the last 4 digits of the Social Security Number of the patient in the field provided.
  10. Under the Authorization section, click on the box “I have read, understand and agree to the Terms of Use and Web Privacy Statement.”
  11. Click on the “CREATE ACCOUNT” button.
  12. Proceed to move forward and provide all the required information to complete the registration for a LabCorp Patient Info online account.
