  3. Select the correct Proxy form you need such as:

    • On behalf of an adult family member

  1. Visit: www.deancare.com/mychart on your device. NOTICE: ONLY BILL PAY is still available on this site until Nov. 1, 2019 for service charges incurred before Sep. 22, 2018.   2. Type in your username and password in the fields provided.
